Why it matters

Why medical insurance matters

Healthcare in Hong Kong is world-class, and it is also expensive. Even routine procedures climb quickly once specialist fees, diagnostics, hospital room charges, operating theatre costs, medication and follow-up treatment are added together. For expats and international families using private hospitals, that can mean real financial stress.

Medical insurance protects you from these risks. Without adequate cover, a single illness, accident or pregnancy can leave you with large out-of-pocket costs, especially when treatment goes beyond a short hospital stay or needs ongoing care.

Types of cover

What types of medical insurance may you need?

International medical insurance

Broad inpatient and outpatient protection, often with worldwide cover, higher annual and lifetime limits, cashless billing and the freedom to choose your doctors and hospitals.

Hospital and surgical insurance

Covers hospital admissions and surgery, including operating theatre fees, surgeon and anaesthetist charges, room costs and inpatient treatment.

Top-up coverage

Sits above an employer scheme or basic plan, raising annual and lifetime limits to reduce your exposure to large hospital bills.

Maternity and newborn cover

Pregnancy-related costs such as prenatal care, delivery and hospital stay, and in some cases complications and newborn cover, subject to waiting periods.

Critical illness insurance

A lump-sum payout for serious conditions such as cancer, to help with treatment costs, recovery and lost income.

How it works

How cover works in Hong Kong

Hong Kong has no universal private cover, so an expatriate family builds protection from three layers: the public system, an employer scheme where one exists, and a personal plan bought on the open market. Most of the questions we are asked come down to where one layer stops and the next has to start.

Almost nobody arrives with all three in place, and almost nobody needs all three at full strength. The work is deciding which layer is carrying the weight in your situation. That answer changes with an employer, a diagnosis or a move.

What actually decides the payout

Nearly every plan sold here works on a reimbursement basis. You are treated, the provider bills you or the insurer directly, and the policy pays back what it covers up to the limit for that item.

The benefit schedule decides the outcome, not the headline sum insured. Two plans with the same maximum can pay very differently once the room rate, surgeon’s fee, anaesthetist’s fee and specialist consultation limits are read against each other, line by line.

VHIS vs international

The VHIS layer, and the tax break.

The Voluntary Health Insurance Scheme (VHIS) is the Government-certified standard that sets a floor under the market. A VHIS Standard Plan carries fixed minimum terms every certified insurer must honour: guaranteed renewal to age 100, no lifetime cap, cover for unknown pre-existing conditions phased in over three years, and cover for prescribed diagnostic imaging and non-surgical cancer treatment.

Those terms are identical whichever certified insurer writes the plan, which makes VHIS the one part of this market you can compare on price alone. Premiums for a certified plan also qualify for a salaries tax deduction, within the annual limit set by the Inland Revenue Department.

It is deliberately a floor, not a ceiling. The limits are modest against Hong Kong private hospital pricing and the scope is Hong Kong, so an expatriate expecting treatment in Singapore, the United Kingdom or the mainland usually needs an international plan above or instead of it.

Price

The choices that move your premium

Four decisions do most of the work on price, and a broker can move all four.

  • Age at entry. Premiums step up in age bands, and the band you join on follows you. Taking a plan out earlier is materially cheaper across the life of the policy.
  • Geographic scope. Adding the United States to an international plan is the single largest increase available. Dropping it is usually the fastest saving.
  • The deductible. Accepting an amount you settle before the insurer pays reduces the premium substantially. It suits a household with savings behind it and an employer scheme covering the smaller bills.
  • The benefit tier. Ward, semi-private or private room entitlement runs through the whole schedule, not just the bed. Moving one tier down often costs less in practice than people expect.

Outpatient, dental and Chinese medicine

Hospitalisation cover is the core of every plan. Outpatient cover is nearly always a separate module. If you want GP visits, specialist referrals, physiotherapy, diagnostics and traditional Chinese medicine to be claimable, it has to be selected and paid for. It is the benefit clients most often assume is included, and most often is not, so price it both ways before you decide.

Our simple process

Four steps, and we stay with you after them.

  1. Talk to us about your needs

We learn your situation, priorities and budget.

  2. Compare your options

We put the three or four best-fit policies side by side, in plain language.

  3. Get covered quickly

We handle the paperwork and deal with the insurers for you.

  4. Ongoing support

When it is time to claim, renew or change your cover, we are there to help.

What we do for you

We are an independent broker, not an insurer. The insurer you choose pays for our services, so our advice costs you nothing. We read the benefit schedules of Bupa, Cigna, AXA, April International, Allianz and Now Health International against each other and put two or three genuine options in front of you. When you claim, we handle the insurer on your behalf.

What clients say

Real reviews from expats, families and businesses who use Expat Insurance for clear advice and reliable support.

“I have only good things to say about Expat Insurance. Insurance is the one thing I buy hoping to never use, but, a Pit Bull experience in Thailand made me thankful of the good policy I bought from Expat Insurance. Hospital costs and new flights were 100% covered but most useful was the ongoing help I received from my agent, Carl. Thank you!”

“Excellent, excellent, excellent. 5 stars to Expat Insurance Hong Kong. My husband and I bought medical insurance top-up plans to work in tandem with our employer scheme. We took out a similar plan with two different insurers and made significant savings on options we had looked at online. Plus, we also got dental coverage! Their practice of face-to-face meetings helped immensely, and it is much nicer to meet with friendly, professional, experienced staff than to do it all online at a greater cost!”

“Marc was very patient with my questions and offered a good selection of affordable health insurance choices. Great service.”

FAQs

Frequently asked questions

Is medical insurance compulsory in Hong Kong?

No. Public hospitals are open to Hong Kong Identity Card holders at subsidised rates, so private cover is a choice, not a legal requirement. What it buys is admission on your own timetable, a choice of specialist and a private or semi-private room.

What does a medical insurance plan cost in Hong Kong?

It depends on age, benefit tier, deductible and geographic scope, so a single figure would mislead. The gap between the cheapest and the most comprehensive plan for the same person is wide, and those levers move it more than switching insurer does.

Should I take VHIS or an international plan?

It may suit you to hold both. Where the budget only stretches to one, the deciding question is normally where you expect to be treated. The scheme terms are published at vhis.gov.hk.

Will a pre-existing condition stop me getting cover?

Usually not. A declared condition is more often a restriction than a refusal: an exclusion, a loading on the premium, or a waiting period before that condition is claimable.

How long before I can claim on a new policy?

Most plans apply a general waiting period of around thirty days from the start date, with longer periods for specified conditions and considerably longer for maternity. This is the commonest reason a first claim is declined.

My employer already covers me. Do I need anything else?

Group schemes are often capped well below Hong Kong private hospital pricing and end on the day the job does. A personal plan, or a top-up above the group benefit, keeps the cover portable and the insured rights in your name.

What does using an insurance broker cost me?

Nothing. We are paid by the insurer you select, at the same premium you would pay going direct. Still deciding? Talk to one of our advisers. No charge, no obligation.
